C.H.O.I.C.E.S.  Behavioral Health Care is an agency providing Substance Abuse and Behavioral Health services to adults, youth and families in our community.

Engaging clients in their community
A key to successful client interaction is meeting them in their environment. C.H.O.I.C.E.S. will engage youth wherever needed, whenever required, at home, school and other community settings.

 
Curriculum
C.H.O.I.C.E.S. Behavioral Health Care offers a variety of psycho-educational curricula that encompass evidence-based and strength-based models. Programming is catered to the individual needs of those served.

Admission process
C.H.O.I.C.E.S. will have an initial discussion with the caller to review admission with the caller to review admission criteria and establish a time for an intake assessment with the caller if an adult, or with the youth and their parent or guardian.


C.H.O.I.C.E.S. will strive to have all intake appointments scheduled and complete within 24-72 hours.


- Please plan on at least two hours to complete the intake interview.
- Picture ID, social security card and insurance card(s) are required at interview.
- If custody is other than a parent, documentation of status is required.​



*The client can usually attend treatment groups within 1 to 3 days after intake.
